Cat Vouchers

This Waiver and Release is made by [YOUR NAME HERE] and is dated [DATE]. I have chosen to participate in The Cat House on the Kings spay/neuter voucher program. I fully understand and acknowledge that my pet and/or community animal identified on each voucher issued to me (“Voucher Animal(s)”) will undergo a surgical sterilization procedure and that following the surgery my Voucher Animal(s) will no longer be able to reproduce. I also understand and acknowledge that, as a service to me, The Cat House on the Kings may transport my Voucher Animal(s) between the drop off location and the place where the surgery will be performed. I further understand and acknowledge that the sterilization/microchipping/vaccination procedures and the transportation of my Voucher Animal(s) have inherent risks, dangers and hazards including, but not limited to, risks associated with surgery, post-surgical infection, automobile accidents, and other risks of injury or death.

I hereby certify that I am the lawful owner of, or an agent authorized to make medical decisions for, the animal described on each voucher. I understand that the veterinarian who is contracted to perform my Voucher Animal’s sterilization surgery may not perform a complete medical examination before surgery is performed. I confirm that I will follow the instructions provided to me with my voucher(s) and at the time of the surgery appointment for the care of my Voucher Animal(s) before and after surgery. I understand that some factors significantly increase surgical risk including, but not limited to, pregnancy, being in heat, obesity, advanced age and diseases such as FIV, Feline Leukemia and heartworms.

I certify that my Voucher Animals are current on their vaccinations or I waive the right to protect my animals by having them vaccinated, or will request that recommended vaccinations be given at the time of surgery. I understand the inherent risks of failing to maintain current vaccinations and waive all claims arising out of or connected with the performance of this surgery due to such a failure. I understand it takes 2 weeks for vaccinations to protect my Voucher Animal(s) and that booster vaccinations are required. Unless otherwise specified on the voucher, vaccinations will be at my expense.

I understand that if any Voucher Animal is pregnant, the pregnancy will be terminated during sterilization surgery and there may be an additional charge. I understand that if a Voucher Animal is found to have an undescended testicle(s), or is lactating, in heat, or obese, sterilization will still be performed and there will be an additional charge for which I will be responsible. I understand that if any Voucher Animal has an open umbilical hernia, it will be repaired at the time of surgery and there will be an additional charge. I understand if any Voucher Animal is found to have fleas, ticks, ear mites, or intestinal parasites, the condition(s) will be treated at my expense.

I understand that, pursuant to California law, if I do not pick up my Voucher Animal(s) at the agreed upon time that The Cat House on the Kings team may exercise its right either to surrender the Voucher Animal(s) to the nearest humane society or animal shelter or to dispose of them as deemed just and proper. I further understand that I will be charged a boarding fee if I do not pick up my Voucher Animal(s) at the time specified at the time of drop off.

I confirm that I have read and understood the provided explanation of the risks and benefits of these procedures and I enter into this contract willingly with The Cat House on the Kings. I understand that the veterinary clinic will provide discharge instructions for the care of my Voucher Animal(s) and what to do and look for before and after my Voucher Animal(s) procedure(s) and confirm that I will follow those instructions. I understand that it is my responsibility to ask any question I may have or request further explanation of any instruction I have received prior to leaving the veterinary clinic or other pick-up location.

I, on behalf of myself, my personal representatives and my heirs, assume all risks and responsibility for all losses, injuries and/or damages which my Voucher Animal(s) may suffer or incur and I voluntarily release, waive, discharge, hold harmless, defend and indemnify The Cat House on the Kings, the veterinary personnel performing the sterilization procedure, and their respective agents, heirs, and employees from all claims, actions or losses for injury to or death of my Voucher Animal(s) and for any related property damage, loss of services or other loss which may arise out of my participation in The Cat House on the Kings spay/neuter voucher program. I specifically understand that I am releasing, discharging and waiving any claims or action that I may have presently or in the future against The Cat House on the Kings and the veterinary personnel performing the sterilization procedure and their respective agents, heirs and employees and acknowledge that the service that will be provided to me would not be available but for this Waiver and Release.
